Does anyone have experience indexing a book or other fairly long publication in InDesign CS4 (or even an earlier version of InDesign)?

One thing I'd like to find out is if we import book chapters into CS4 from PgMkr 7, will the PgMkr index entries be imported and if so, how do we access/edit them, and will we be able to generate an index from the imported PgMkr entries in CS4?
